Just a few thoughts.

I loved that the first thing we see when Dean wakes up is the amulet. He almost never takes it off, does he? It's just a wonderful sweet image of how they used to be so much closer.

Damn it Sammy, wake up and smell the roses! You know Dean's angry at you, and you know that everything he said back at that hotel room was completely true. So talk about it! Don't just do the 'We're all fine' thing! You're supposed to be the open one!

Another bit of amulet-related imagery (wut? The amulet is really the first thing I notice a lot). During the fight, the amulet looks like it's almost coming off at one point. Not sure if that's intentional, but it really made me worry like heck.

And the man playing the Siren-as-Ned made it really, really creepy. He did a good job of it. Really, really good.

Oh, boys. You are not fine. You are so screwed to hell and back again.
